At our initial appointment we will answer any questions you have, review HIPAA and confidentiality standards, and determine what the goals for our involvement with your family will be. Based on our initial assessment and meeting, we will develop an individualized ACM Care action plan. It will outline the services we will provide, and how often these services will be delivered.
